What to expect on your first visit

When scheduling your appointment, you will receive an email with a link to our Patient Portal. Prior to your first visit please login to the patient portal and complete your profile. Please contact our office if you need help logging into your patient portal. If you are unable to complete the portal you will need to arrive 15 minutes early to allow enough time to complete the portal using our patient kiosk.

Upon check-in our front office staff will also need to scan your insurance card (if you have insurance) and due to heightened identity theft, we will need a verification of identity with a photo I.D.

If you were referred to our practice by another doctor’s office, please inform our front office staff so we may send the referring office a consultation report.

Additional Information about Our Office

We understand your time is valuable and we will make every effort to see you at your scheduled appointment time; however, sometimes unforeseen problems can arise, and you may have to wait longer than expected. Our office is dedicated to giving our patients the utmost in care and service, so be assured that we will give you the same careful attention as soon as possible.

It is our office policy to only treat patients under the age of 18 with a parent/guardian consent. A child must be accompanied by the parent or guardian to his or her first office visit; however, the child may come to subsequent visits without a parent present if a written consent is provided by the parent or guardian giving our office permission to treat the child without them being present. If you do wish to send your child to his or her appointments alone (once consent is on file), please ensure they have payment for any charges that may be incurred at the visit.

About Telephone Calls

Our front office staff has been trained to answer many of the questions you may have; however, if you have a medical question or concern, these calls must be taken by our trained medical staff and physicians. Because of this, the front office staff will transfer those calls to our triage line. If your call is answered by voicemail, please leave a message stating your concern and your call will be returned by the end of the day. In the case of a concern that cannot wait our front office staff will get you immediate help.

If you need a medication refilled, please contact your pharmacy directly to request more refills. Your pharmacy will then fax our office a request on your behalf. Please request more refills at least seven days prior to running out of your medicine as to allow time for any issues that may arise. It is important to note, that depending on when you were last seen by our physicians, they may request you make a follow-up appointment before authorizing more refills. This is to ensure you are using the best and most effective therapy possible and to re-evaluate the status of your condition.

About Insurance Plans, Fees and Payments

We are contracted with most major insurance plans, Please contact our office for the most up-to-date information on the plans we are contracted with.

Please note co-payments, co-insurance, or other portions of the bill that are not paid by the insurance company will be collected at each visit. Our billing department can arrange a payment plan if necessary, however, these arrangements must be made in advance.
