At Alliance Dermatology and Mohs Center we try very hard to provide our valued patients with the highest level of service offered in the medical field. What to expect on your first visit
On your initial visit to our office, please be prepared to fill out a few pages of paperwork. These patient intake forms are available on our website, and you may print these out, fill out at your convenience, and bring them in for your initial appointment. If you do not wish to print these forms yourself, our office can provide them to you. If you elect to fill out the forms in our office, please arrive 15 minutes ahead of your scheduled appointment as to allow sufficient time for you to complete these forms.
In addition to retrieving your completed forms, our front office staff will also need to make a photocopy of your insurance card (if you have insurance). This is necessary to ensure we have the correct information to bill for any services. If you do not have your current health insurance information, we will not be able to submit a claim on your behalf and you will be responsible for all charges on the day services are rendered.
Due to heightened identity theft, we will need a verification of identity with a photo I.D. as well. Please be prepared to provide photo identification, such as driver’s license, for our staff to make a copy of.
It is very important for our medical staff to know as much as possible about your health. Please be prepared to provide a brief health-related history, including any previous skin cancer, your current medications, any medication allergies you may have, and any other details that could be beneficial in treating you.
If you were referred to our practice by another doctor’s office, please inform our front office staff so we may send the referring office a consult report.

Monday & Tuesday: 1:30pm-4:30pm
Sun City West Office:
Monday, Tuesday & Wednesday: 8am-11am
Sun City West Office:

Monday & Tuesday: 8am-5pm; with lunch taken between 12pm-1pm
Wednesday & Thursday: 8am-5pm; with lunch taken between 11:30am-12:30pm

Additional Information about Our Office
We understand your time is valuable, and make every effort to see you at your scheduled appointment time; however, sometimes unforeseen problems can arise and you may have to wait longer than expected. The fact is our office is dedicated to giving our patients the utmost in care and service, so be assured that we will give you the same careful attention as soon as possible.
While waiting in our reception area, it may appear that some patients are being called back out of turn; however, we see each patient in the order of which they are scheduled, not in the order of which they have signed in. Some patients are also here for non-physician visits so this may also impact when someone is taken back.
It is our office policy to only treat patients under the age of 18 with parent/guardian consent. A child must be accompanied by the parent or guardian to his or her first office visit to complete the new patient intake forms; however, the child may come to subsequent visits without a parent present if a written consent is provided by the parent or guardian giving our office permission to treat the child without them being present. If you do wish to send your child to his or her appointments alone (once consent is on file), please ensure they have payment for any charges that may be incurred at the visit.
About Telephone Calls
Our front office staff has been trained to answer many of the questions you may have; however, if you have a medical question or concern, these calls must be taken by our trained medical staff and physicians. Because of this, the front office staff will transfer to our medical staff for assistance. Please be advised that our medical assistants and physicians are with patients throughout much of the day and are unable to take phone calls except for emergencies only. All other calls will be transferred to our voicemail system, which is checked periodically throughout the day. Please be aware that at least one attempt will be made to return your phone call before we leave for the day. In situations where we are unable to reach you on the same day, we will make additional attempts on the following business day to reach you.
If you are in need of a medication refill, please contact your pharmacy directly to request additional refills. Your pharmacy will then fax our office a request on your behalf. Please request additional refills at least seven days prior to running out of your medicine as to allow time for any issues that may arise. It is important to note, that depending on when you were last seen by our physicians, they may request you make a follow-up appointment before authorizing additional refills. This is to ensure you are using the best and most effective therapy possible and to re-evaluate the status of your condition.
About Insurance Plans, Fees and Payments
We are contracted with most major insurance plans, please contact our office for the most up-to-date information on the plans we are contracted with.
Please be advised that any co-payments, co-insurance, or other portions of the bill that are not paid by the insurance company will be collected at each visit. Our billing department is able to arrange a payment plan if necessary, however, these arrangements must be made in advance.
